It’s time again! We’re looking for five youths (under age of 21) with a chronic condition(s), who show extraordinary leadership and contribute to their school and community.
Please help us by nominating the young person that comes to mind. The process is simple; fill out the Nomination Form, telling us about them and how their efforts and/or leadership have made an impact in their community. Submission Deadline: Monday, June 19th
Awards given at CYCC Awareness Event; McAuliffe-Shepard Discovery Center, July 13th from 5:00 to 7pm. Winners will be announced before the event, so you can plan for attending the event.
